Health
The mission of CitySquare’s health programs is to improve access to care and the most critical health needs facing South Dallas’ residents.
Community Clinic
Since 1998, the CitySquare Community Clinic has served
adults and children by providing a permanent medical home
for the uninsured in our community. Through preventative
screenings, a collaborative care team approach, and
innovative community-based practices, the CitySquare
Community Clinic became recognized in 2012 as a National
Committee for Quality Assurance (NCQA) Level 3 Patient
Centered Medical Home – the highest level attainable.

Primary Care: Receive ongoing care through a primary care doctor for both adults and children, including preventive screenings and immunizations.
Behavioral Health: Provide access to screening for depression, anxiety and substance abuse, and help to address problems from food insecurity to behavioral health needs. A Licensed Medical Social Worker (LMSW) provides ongoing behavioral health counseling for improved functioning and well being, and makes referrals to community resources as needed including for substance abuse counseling and treatment.
Chronic Disease Care: Provide ongoing support and education for those with diabetes, high blood pressure, asthma, chronic obstructive pulmonary disease (COPD), and tobacco cessation.
Medication Management: Provide ongoing support to ensure appropriate use of medications through onsite education and medication adherence calls.
Referrals for Specialty Care: Create connections to resources for specialty services such as x-rays, specialty doctors, and surgeons through partnerships with Baylor Scott and White Health Care System and Parkland Hospital.

Cost:
Medical visits are $10 per appointment. There is no fee for the enrollment visit.
New patients must provide:
Proof of Income, Proof of Residency, Identification (Valid Photo ID)
