Community Health Program
Our Community Health Promotion Program helps neighbors adopt healthy habits and prevent the chronic diseases often prevalent in lower-income communities.

Neighbor Health Support
Neighbor Health Support (NHS) provides health and wellness navigation and support to CitySquare neighbors in existing programs. NHS has a community health nurse, community health workers and health navigators that provide health education, navigation services and prevention screenings.

Community Health Workers Community Health Outreach
The Community Health Outreach (CHO) team also organizes an annual mobile vaccination clinic to provide neighbors and staff with tuberculosis testing and vaccinations for the flu, COVID-19 and Hepatitis A. To promote healthy activities, CitySquare participates in health fairs and conducts outreach efforts in schools, churches, local businesses and housing sites. Additionally, CHO staff work within the community to enroll the uninsured into health insurance, provide health education and connections to resources, and link individuals to a primary care provider.
