Outreach and Case Management with homeless youth, requiring knowledge of and experience in Mental Health, Addictive Disorders, Trauma Informed Care and Benefits Eligibility, especially as it relates to the homeless. The position involves engaging and building a relationship with youth who might be on the streets, couch surfing, and in encampments or abandon buildings. The Youth Homeless Outreach Case Manager will begin to coordinate access and screen youth for entry into youth housing programs in this area. This position will be working closely with Neighborhood Support Services, DOPS, and HMIS. This position will also have to partner with other youth outreach programs in Dallas, including the ISDs, police & other security departments, as well as partner non-profits. Case Manager will be responsible for helping homeless youth to achieve successful housing and provide ongoing support.
Education: Bachelor’s degree; Social Work or related field preferred.
Experience: One (1) year experience in case management working with homeless population required; experience working with young at-risk populations, youth in foster care or who have been neglected or abused highly desirable.
Licensure/Certification: None required. Current Texas Social Work license preferred.
Additional Requirements: Must be proficient in MS Word, Excel and Outlook. Must be able to utilize a case tracking system. Must be available to work evenings, weekends, and on-call (24 hrs). Must have valid driver’s license and reliable transportation. Must be able to travel within assigned region on a daily basis.
