The TRAC Aftercare Services/Case Manager will provide case management services, support services, and independent living skills training to current and/or former youth ages 18 and older transitioning from substitute care to independent living. He/she will provide supportive counseling; linkage to resources; housing, employment and independent living training; crisis intervention and management, among other services, to participants in this program.
- Education: Bachelor’s degree required; degree in social work or related field preferred. Master’s in Social Work (MSW) highly desirable.
- Experience: Two (2) years of experience working with children or youth who are at-risk, in foster care or who have been neglected or abused required.
- Licensure/Certification: None required.
- Additional Requirements: Must be proficient in MS Word, Excel, and Outlook. Must be proficient in managing a case tracking system. Must be available to work evenings, weekends, and on-call (24 hours). Must have driver’s license and reliable transportation. Must be able to travel within assigned region on a daily basis.
