Health

The mission of CitySquare’s health programs is to improve access to care and the most critical health needs facing southern Dallas’s residents. At CitySquare, we believe that health is determined as much or more by one’s social environment as by one’s genetics. We understand that social determinants of health, conditions in the environments in which people are born, live, learn, work, play, worship and age, are a critical part of one’s health and affect a wide range of health, functioning and quality-of-life outcomes. That’s why we offer four separate programs to keep uninsured folks in our community healthy and active.

CitySquare Community Clinic

Since 1998, the CitySquare Community Clinic has consistently served adults and children by providing a permanent medical home for the uninsured in our community. Through preventative screenings, a collaborative care team approach and innovative community-based practices, the CitySquare Community Clinic became recognized in 2012 as a National Committee for Quality Assurance (NCQA) Level 3 Patient Centered Medical Home – the highest level attainable.
