This Lead Case Manager will work in the Walk-In Case Management program of the NSS Department. The Walk-In Case Management program is often the first stop for neighbors seeking support and is the gateway for neighbors to be served throughout CitySquare. Neighbors visit Walk-In Case Management to receive triage, referrals, or intensive case management. This position is responsible for overseeing Walk-In Case Management programming and training and supervising AmeriCorps members and interns. The Lead Case Manager will be responsible for a small case load and will support their staff of AmeriCorps members and interns to ensure they are fully trained to provide case management, provide referrals, conduct follow up as needed, and maintain and inform neighbors on available community resources. She/He/They will coordinate supportive services with individuals and families who are homeless, formerly homeless, at risk of becoming homeless and/or low-income. While this position will be officed at the CitySquare Opportunity Center, they will also provide supportive residential services and activities for residents living in affordable housing at Belleview and Creekside apartments twice/week.
Education: Bachelor’s degree in Social Work; Master’s degree in Social Work preferred.
Experience: One (1) year experience in case management & working directly with vulnerable and homeless populations required; Three (3) years experience preferred. Orientation to best practices in case management services for vulnerable populations.
Licensure/Certification: Current Texas Social Work license preferred.
Additional Requirements: Excellent communication and critical thinking skills. Positive attitude, flexibility in a highly dynamic environment and able to handle crisis situations under pressure. Self-motivated with ability to take initiative and be resourceful. Ability to prioritize and manage multiple projects/tasks. Must be proficient in MS Word, Excel, and Outlook. Ability to work occasional flexible hours as requested. Must have a valid driver’s license, reliable transportation with valid insurance. Willing to transport program participants, as needed. Willing to work with chronically homeless individuals with disabilities (physical, mental and/or substance abuse) who may or may not have criminal backgrounds. Bilingual-Spanish speaking preferred.
