The TRAC Lead Case Manager is responsible for program implementation and coordination of the AfterCare Case Management & Services, including intake, assessment, referral, person-centered planning, mentoring, case management and skill building. This position collaborates with all stakeholders of this North Texas collaborative composed of numerous community providers and state and private agencies to manage a comprehensive case planning approach; maintaining a high degree of external contact with community resources. He/she will continue to provide case management while overseeing a team of other Aftercare Services Managers. The Lead Case Manager will be responsible for ensuring that case management provision meets contract guidelines and expectations and will be responsible for completing necessary reports. He/she will work closely with the TRAC Director and Asst. Director to also assure that all services meet quality guidelines and are tracked on a regular basis for reporting and analyzed for program improvement.
Education: Bachelor’s degree required; advanced degree in social work or a related field preferred.
Experience: At least 3 years’ experience working with youth in foster care via Child Protective Services, other child placing agency, or community partner. Demonstrated history of exemplary organizational, management and communication skills.
Licensure: None required, but preferred.
Additional Requirements: Must be proficient in MS Word, Excel and Outlook. Must be able to utilize a case tracking system. Must have valid driver’s license and reliable transportation. Must be able to travel within assigned region. Must be able to work a flexible schedule including nights and weekends as required.
