Homeless Outreach Case Manager (ISOP grant)

Outreach Case Management with unsheltered homeless persons through Intensive Street Outreach. This position requires knowledge of and experience in Mental Health, Addictive Disorders and Benefits Eligibility, especially as it relates to the homeless. The position involves providing assessments with homeless neighbors for mental health/substance abuse treatment and case management services. The Homeless Outreach Case Manager will also assist neighbors with acquiring critical documents, mainstream benefits, as well as housing referrals.

Education: Bachelor’s degree in Social Work or related field.
Experience: Three (3) years experience working with homeless population.
Licensure/Certification: Current Texas Social Work license preferred.
Additional Requirements: Must be proficient in MS Word, Excel, and Outlook. Ability to work occasional flexible hours as requested. Must have a valid driver’s license and reliable transportation. Also, must be able to drive the Mobile Outreach Vehicle (RV).
