The Clinical Case Manager will provide a combination of counseling and intensive case management to formerly homeless individuals in CitySquare’s Community Life program, a Permanent Supportive Housing (PSH) Program, requiring knowledge and experience in Housing First, Mental Health, Addictive Disorders, and Benefits Eligibility. The Clinical Case Manager will assist participants in communication with and support from established agencies, charities, and government programs, including but not limited to CitySquare. Additionally, the Clinical Case Manager will provide a minimum of four hours of counseling per week to neighbors in need of behavioral therapy using a person-centered, strengths-based approach that is sensitive to and knowledgeable of Harm Reduction and Housing First Models. (Clinical supervision will be subsidized.) In addition, the Clinical Case Manager will provide psychoeducational and therapeutic support groups. The Clinical Case Manager is also responsible for helping participants achieve successful placements and provide ongoing support on-site to those residents living at CityWalk apartments. The overall goal is to help participants to continue to live independent, successful lives in their own apartments.
Education: Master’s degree in Social Work, or a related field, is required.
Experience: One-Two (1-2) years’ experience in case management, experience working with the homeless preferred.
Licensure/Certification: LMSW (and working on clinical hours for LCSW licensure) or related accreditation required.
Additional Requirements: Must be proficient in MS Word, Excel and Outlook. Ability to work occasional flexible hours as requested. Must have a valid driver’s license and reliable transportation. Willing to work with chronically homeless individuals with disabilities (physical, mental and/or substance abuse) who may or may not have criminal backgrounds.
