The Case Manager for Neighbor Support Services will provide intensive case management and supportive services to chronically homeless individuals and families housed in the City of Irving’s Permanent Supportive Housing (PSH) program. Case manager will require knowledge and experience in housing first, mental health, addictive disorders, and benefits eligibility. Case manager is responsible for ensuring program compliance and helping participants achieve successful placements and provide ongoing support to those enrolled in PSH. In addition to providing housing-based intensive case management through the City of Irving’s Community Development Block Grant, this position will also be responsible for providing housing focused case management that includes working with homeless individuals at CitySquare’s Opportunity Center to work on collecting Documentation of Prioritization Status (DOPS) to get them placed on the Housing Priority List (HPL). Case Manager will take referrals from Walk-In Case Managers and other CitySquare programs.
Education: Bachelor’s degree in Social Work, or a related field, is required. Masters preferred.
Experience: One (1) year experience in case management, experience working with the homeless preferred.
Licensure/Certification: Licensed from an accredited university in social work preferred.
Additional Requirements: Must be proficient in MS Word, Excel and Outlook. Ability to work occasional flexible hours as requested. Must have a valid driver’s license and reliable transportation. Willing to work with chronically homeless individuals with disabilities (physical, mental and/or substance abuse) who may or may not have criminal backgrounds.
