Case Manager for the A Way Home Program

Position Summary
The A Way Home (AWH) is a Landlord Subsidized Leasing Program (LSLP). The AWH Case Manager will provide field-based case management for homeless individuals. The AWH Case Manager will use Motivational Interviewing to assist and support the participants in addressing the barriers that led them into homelessness. Requires knowledge and experience in Housing First, Mental Health, Addictive Disorders, Benefits Eligibility, strength-based models, and Job Coaching. Provide supportive contacts and assist clients in navigating through the community and assist participants with referrals from established agencies, charities, and government programs. Case Manager is responsible for helping participants maintain successful placements and provide ongoing support to those enrolled in the program. The primary goal is assisting the participant to live independently in their own apartment.


  • Education: Bachelor’s degree in Social Work, or a related field, is required. Masters preferred.
  • Experience: One (1) year experience in case management, experience working with the homeless preferred.
  • Licensure/Certification: Licensed from an accredited university in social work preferred.
  • Additional Requirements: Must be proficient in MS Word, Excel and Outlook. Ability to work occasional flexible hours as requested. Must have a valid driver’s license and reliable transportation. Willing to work with chronically homeless individuals with disabilities (physical, mental and/or substance abuse) who may or may not have criminal backgrounds.
