Case Manager (Destination Home)

Position Summary
Case Manager will provide case management to chronically homeless individuals in CitySquare’s Destination Home, a Permanent Supportive Housing (PSH) Program, requiring knowledge and experience in Housing First, Mental Health, Addictive Disorders, and Benefits Eligibility. Case manager will assist participants in communication with and support from established agencies, charities, and government programs. Case Manager is responsible for helping participants achieve successful placements and provide ongoing support to those enrolled PSH living in scattered site apartments. The overall goal is to help participants to continue living independently in their own apartments.

Qualifications

  • Education: Bachelor’s degree in Social Work, or a related field, is required. Masters preferred.
  • Experience: One (1) year experience in case management, experience working with the homeless preferred.
  • Licensure/Certification: Licensed from an accredited university in social work preferred.
  • Additional Requirements: Must be proficient in MS Word, Excel and Outlook. Ability to work occasional flexible hours as requested. Must have a valid driver’s license, reliable transportation with valid insurance. Willing to transport program participants, as needed. Willing to work with chronically homeless individuals with disabilities (physical, mental and/or substance abuse) who may or may not have criminal backgrounds.
